Utfärdat samt egenhändigt signerat av konung Carl XIII 22 november 1810, för Archiatern och professorn vid universitetet i Greifswald Lorenz Wilhelm Haselberg (1764-1844).
Inbunden i rött skinnband präglat samt förgyllt.

Skinnbandet med omfattande skador, inlagan med fläckar samt ett litet hål.
